Start Now:

• FIRST - Register for your expedition

• Get Passport (if you don’t have one)

• Get vaccinations as recommended by your health care provider and the Center for Disease Control 

• "Like" Humanitize Expeditions on Facebook

• Gather donations and supplies

• Book your flight.

• Submit/upload Passport, flight information, Medical Evacuation Insurance information, medical license (if applicable), parent permission form (under 18).

• Print out and sign the following forms.​

  • Sign and turn in Liability Waiver and Risk Statement

  • If under 18 years old, turn in the Parent Permission Form

  • Sign and turn in the Code of Conduct

The original copy of these forms can be turned into your Team Leader or Mailed to our PO Box.
